The Department is currently facing an unprecedented challenge in the face of the public health emergency arising from COVID-19. In light of this, it is possible that the Department may not be able to respond to some Subject Access Requests within the normal deadlines. The Department will be in contact with you should this be the case in relation to your request.

To make a Subject Access Request (SAR), please download the Subject Access Request Form and email the completed form to or post to:

Proof of identity must accompany this Subject Access Request form.
DownloadRequests for personal data must be accompanied by proof of you identify, including a photocopy of your passport or driving licence or Public Services Card, so that we can verify your identity.
Requests for personal data should include:
- your name, PPS number, and postal address
- your signature and the date of the request
- if you are applying on behalf of another person, then a signed authority from the individual is required
- any details which may help us locate the information which is the subject of your request
